IFO CONFLICT OF INTEREST DISCLOSURE STATEMENT
This disclosure statement is to be completed within 60 days of selection to serve in office and thereafter annually by October 1. The IFO Conflict of Interest Policy does not prohibit people from participation based on potential conflicts of interest, but allows the organization to know when individual participation could be influenced by outside factors. If you have questions or are unsure, you are encouraged to disclose since that evidences good faith on your part.
The Disclosure Statement shall be collected (with assistance from campuses) and maintained in the State IFO Office. Statements will be available for review only by the appropriate campus Faculty Association President and the IFO President. The State IFO President may authorize disclosure to others for purposes of enforcing the IFO Conflict of Interest Policy. If the IFO President reports a conflict of interest, then the IFO Vice President will also review it. If the FA President reports a conflict, then the FA Vice President will also review it.
Please check and fill out one item below; NO, if you do not have any conflicts to report and YES, if you do have conflicts to report. Please read the policy and questionnaire to determine if you might have reportable conflicts.
If you have any questions please contact Donna Blake, IFO Controller by email or phone at 651-227-8442 ext 12.
