Join the IFO *

I hereby request and voluntarily accept membership in the Inter Faculty Organization (IFO) and the local Faculty Association on my campus. I authorize the IFO to act as my exclusive representative in collective bargaining over compensation, benefits, and other terms and conditions of employment in writing during the window period, irrespective of my membership in the IFO.

Dues Authorization: Payroll Deduction *

I hereby request and voluntarily authorize my employer, the State of Minnesota, to deduct periodically from my earnings as a MnSCU University employee an amount equal to dues applicable to IFO members for the academic year. This authorization shall remain in effect and shall be irrevocable unless I revoke it by sending written notice by U.S. Mail to the IFO, postmarked during the window period of April 1 to June 15. This authorization shall renew and remain irrevocable from academic year to academic year unless I revoke it in writing during the above-described window period, even if I have resigned my membership in the IFO. I understand that if I make a lump sum payment of the full year’s membership dues to the IFO, there will be no deduction of membership dues from my pay for that year.
